To maximize our patient's convenience as much as possible, ACT will direct bill your insurance on your behalf. Our billing representatives work to get your claims paid with as little intervention as needed.

We can accept medical records requests from an authorized individual. The request form may be faxed or mailed only. Fax or mail to the address below.
Payment Processing
We accept cash, check, credit card*, or debit card payments. Please note that there is a service charge when processing credit card payments.

Our billing department may process a one-time or recurring payment.

We now accept online payments. You may start your online payment using the "Make a Payment Online" button on the top of this page. Please make sure you include the patient's last name and first initial and the chart number (included on your statement). If you need your chart number, our billing team will be able to assist you at 708-305-0562.
